Loss of Iranian Passport

 

Apply for loss of passport

(For those whose passports have been stolen or lost)

 

Required documents to issue a new passport:

1- Completing the passport application form on the mikhak site and receiving the tracking code.

2- Completing the missing report form at the Consulate General

3- The report of the local authorities (police department) about the loss of the passport. Please note that the police report must be on the police letterhead and with the police stamp.

4- Original sshenasnameh and national card

  • It is necessary to have a shenasnameh with a photo for people over 15 years old.
  • If the sshenasnameh is in Iran, the relatives of the can go to the Ministry of Foreign Affairs in Tehran (Imam Khomeini Square, Building No. 3, Passport Office and visas, resident department) or the provincial offices of the Ministry of Foreign Affairs in most major cities such as Shiraz, Isfahan, Tabriz, Bushehr, Mashhad, etc. The tracking code or letter number from Iran should be sent along with the documents.

5- Valid proof of residence in India such as a visa or OCI

6- Copy of missing passport (if any)

7- Two pieces of new colour personal photos (related to a recent week) full face with white background, without glasses, hat and scarf. The photo of women over 9 years old should be in hijab.

8- The original  card of service or exemption of military service card (for men)
